Hands-On Clinical Care & Daily Health Support

Our Licensed Practical Nurses (LPNs) provide direct, hands-on medical care and routine health monitoring for clients who need ongoing clinical support at home. As an essential part of the care team, LPNs work closely with Registered Nurses (RNs), physicians, and family members to ensure each client receives safe, effective, and compassionate care.

LPNs not only assist with daily clinical tasks, but also advocate for the patient’s comfort, dignity, and overall well-being. Their role supports both the client and their family, ensuring that care is consistent, coordinated, and tailored to health needs.

Services may include:

• Routine Nursing Care
  Monitoring vital signs, symptoms, and health changes during each visit.

• Medication Assistance
  Administering or assisting with prescribed medications and treatments.

• Basic Wound Care & Dressing Changes
  Supporting healing and ensuring proper infection control.

• Chronic Condition Support
  Helping manage conditions such as diabetes, respiratory diseases, or mobility limitations.

• Patient & Family Communication
  Updating families and RNs on health changes and care needs.

• Advocacy & Observation
  Ensuring clients receive appropriate care and quality of life at home.
